Approach

This is how you make a request for help to the municipality:

  • Contact the municipality. You may do this yourself, but someone else may also do it for you. For example, a family member or caregiver. Don't have anyone who can help you with the notification? There are organizations that can support you.
  • The municipality will make an appointment with you for an interview. Someone else may also be present at this interview. For example, a family member, acquaintance, neighbor or someone from client support.
  • The council will discuss the following issues with you:
    • What can you still do yourself?
    • What can people around you do for you?
    • Are there municipal facilities that can solve your problem?
  • After the interview, a plan of action is written. Does the plan state that you need an individual tailor-made facility? Then you will submit an application to the municipality. It is sometimes possible to receive a customized service as a personal budget (pgb).
Cost

In Heerlen, you do not pay a parental contribution. Not levying parental contribution may affect child benefit.

Description

Perhaps as a parent/educator or a child under 18, you have questions about such things as: illness, nutrition or problems at home. You may have:

  • your child is truant
  • there is a lot of arguing
  • there are addiction problems
  • your child is depressed or
  • there are other psychological problems

At the Access Team you will receive explanation, advice or help. A staff member examines with you what your request for help is. Then together you look at what you or those around you can do yourself. Family members, friends and neighbors are often willing to help. Perhaps there are facilities in your neighborhood that you can use, such as a parenting course. Often there are volunteer organizations in your area that can help you.

Together with the staff member, you look at where professional help might be needed. You make a plan together. In this plan you record the agreements. This plan is called 1Family1Plan. You can also submit a personal plan. Indicate what help you think you need. You can submit this plan to the social neighborhood team.

Does the investigation reveal that you need an individual or customized facility? For example, youth mental health or addiction care. In the social buurtteams municipal employees are aware of the plan. They make sure you get the right facilities.
