Extract of birth, death, marriage and divorce
An excerpt or transcript of civil status is written proof of, for example, a birth or marriage. You request this from the municipality where the event took place.

Approach
This is how you request an extract or transcript from the civil registry:
- You go to the municipality where the event was. Do you want to apply for a divorce certificate or certificate of termination of partnership? Then do so at the municipality where you were married or where you entered into the partnership.
- Log in with your DigiD at the municipality to request the extract or transcript online.
- You pay for the excerpt or transcript online.
- You will receive the excerpt or transcript at home.
Applying from abroad
Do you live abroad and need an extract?
- Apply online and pay via iDeal or credit card.
- Authorize someone else to apply for you.
You can request a multilingual extract for use abroad. Please make an appointment at the front desk to do so. The information is in the following languages:

Applying for someone else
Are you requesting the extract for someone else? If so, please include:
- A signed letter stating what the applicant needs the extract for and that he/she authorizes you.
- A copy of the applicant's ID.
- Bring your own proof of identity.
