Extract of birth, death, marriage and divorce

An excerpt or transcript of civil status is a written record of, for example, a birth or marriage. You request this from municipality where the event took place.

You can request this for yourself, your partner, parent or child.

Approach

This is how you request an extract or transcript from the civil registry:

  • You go to the municipality where the event was. Do you want to apply for a divorce certificate or certificate of termination of partnership? Then do so at the municipality where you were married or where you entered into the partnership.
  • Log in with your DigiD at municipality to request the extract or transcript online.
  • You pay for the excerpt or transcript online. 
  • You will receive the excerpt or transcript at home.

Applying from abroad

Do you live abroad and need an extract?

  • Apply online (login DigiD) and pay via iDeal or credit card.
  • Authorize someone else to apply for you.

You can request a multilingual extract for use abroad. This will contain your information in the following languages:

  • French
  • German
  • English
  • Spanish
  • Greek
  • Italian
  • Portuguese
  • Turkish
  • Yugoslav 

Applying for someone else

Are you requesting the extract for someone else? If so, please include:

  • A signed letter stating what the applicant needs the extract for and that he/she authorizes you.
  • A copy of the applicant's ID.
  • Bring your own proof of identity.

Cost

A limited review with corrections is free.

To request an extract or transcript from the civil registry, you pay €16.60.

A multilingual extract or model form costs €16.60.

Description

The municipality writes down important events in a person's life in an official document. This is called a deed. Important events include birth, marriage or death. The deeds are kept in the municipality civil registry.

You can request a hard copy or excerpt of these deeds. An excerpt is a summary of the information the municipality has. It contains the most important information. A transcript is a copy of the deed, stamped and signed by the registrar.

You can request a transcript or excerpt from a:

  • birth certificate
  • death certificate
  • marriage certificate
  • divorce certificate
  • certificate of partnership
  • deed of termination of partnership

Do you need an extract for abroad? Then request an international excerpt.

Do you need a statement for abroad? Then request a multilingual model form in addition to the statement.

For the multilingual model form, make an appointment at the front desk. 

Deeds become public over time. For births this is after 100 years, death certificates after 50 years and marriage and divorce certificates after 75 years. You can look up public records yourself at the archives department, Rijckheyt in Heerlen. Looking up the records yourself is free of charge.

Terms

Do you want to request a copy or extract of a deed for someone else? This is only possible if you:

  • be the child or grandchild or be the parent or grandparent (i.e. not a sibling)
  • are an heir
  • are the spouse or registered partner
  • are the legal representative
  • needs the deed for legal proceedings
  • Need the deed for a marriage or registered partnership
  • Has a written consent (an authorization)

The municipality will send the extract or transcript to you within 3 business days. Do you live abroad? Please note that postal delivery may take longer.

Do you need the extract or transcript sooner? Then make an appointment at the municipality. You will receive it immediately during your appointment.
